Hazel Patricia Moder was born on November 28, 2004 in Los Angeles, California, to Julia Roberts and Daniel Moder. Her mother is a renowned Hollywood actress, while her father works as a cinematographer in the entertainment industry. Both her parents were previously married; her mother to country singer, Lyle Lovett, and her father to Vera Steimberg. When her parents first met on the set of the 2000 film 'The Mexican' her father was still married to his previous wife and her mother was dating actor Benjamin Bratt. They got married on July 4, 2002 in Taos, New Mexico, shortly after her father's divorce was finalized. She doesn't have any step siblings, but has a fraternal twin brother named Phinnaeus Walter Moder, a.k.a. Finn, and a younger brother named Henry Daniel Moder, who was born on June 18, 2007. Her uncle Eric Roberts, aunt Lisa Roberts Gillan, and cousin Emma Roberts are all actors. She attends Our Lady of Mercy School in Los Angeles, where she actively participates in various sports events.

Her mother, who is a practitioner of Hinduism and a devotee of late Indian spiritual guru Neem Karoli Baba, took her children to Swami Daram Dev's Ashram Hari Mandir in Pataudi, Haryana in September 2009 during the shooting of her film 'Eat Pray Love' in India. The children were subsequently given new names after Hindu gods, with her name being Laxmi.

Hazel Moder has grown up a bit differently than most celebrity kids of her age. According to her mother Julia Roberts, "she subscribes to the low-key Meryl Streep school of celebrity mothering". Julia had reportedly asked Streep's daughter, Grace Gummer, how she grew up as the daughter of a superstar and applies some of the insights to keep her children away from the spotlight. However, she is aware that her daughter might want to get into acting in the future, and would like to consult the mother of Natalie Portman, who had carefully selected acting assignments for her daughter to make sure that her studies didn't get affected.

Nevertheless, Julia Roberts emphasized that she would rather let her children have a normal childhood, playing on the field, than worry about their future in the industry. As such, Hazel's parents have made sure that the kids are away from the prying eye of the media. She has occasionally been spotted alongside her family members or at her school sports events. Even with the limited opportunities, the tabloids were quick to point out how the young girl is growing up looking more and more like her mother. In April 2017, during an interview with 'People' magazine, Hazel Moder's mother Julia Roberts revealed that she has safely kept one of her most iconic dresses from her red carpet appearances for her daughter Hazel in the hope that she might want it someday. The dress is the black and white Valentino gown that she was wearing while accepting the 'Oscar' for her performance in the 2000 biographical film 'Erin Brockovich' at the '73rd Academy Awards' on March 25, 2001. The dress, a vintage 1992 design from the Valentino archives, was voted as the third most iconic red carpet dress of all time according to a poll by Debenhams, published in the 'Daily Telegraph'. Interestingly, the iconic dress also has its own Wikipedia page. Roberts mentioned that she keeps that dress and many other such significant ones from her long career in a box under her bed, in a little space that her husband "refers to as the heritage collection".
