Scientists and medical experts from ShanghaiTech University, Chinese Academy of Sciences, Eye, Ear, Nose and Throat Hospital of Fudan University, and Shanghai General Hospital have made a significant discovery related to CAR-T immunotherapy. CAR-T, or Chimeric Antigen Receptor T-Cell Immunotherapy, is a biological immunotherapy process that involves taking T cells, a type of immune cells, from the patient and adding a chimeric antibody through genetic engineering. The antibody acts as a navigator to help T cells recognize cancer cells and activate T cells’ ability to destroy cancer cells. These CAR-T cells are then transported back into the patient’s body after being amplified in vitro. Compared to other biological immunotherapy, CAR-T treatment is more precise and targeted to cancer.

However, CAR-T treatment has a weaker effect on solid cancers than blood diseases. The reason behind this is that CAR-T cells can become weak and exhausted as CAR generates a type of tonic signal while working on a solid cancer. The tonic signal works like an order to ask a person to repeat the same task, which exhausts his or her energy and patience. The repeated signals from CAR also cause the fatigue of T cells, whose anti-cancer function fails.

Experts discovered that electrostatic interaction caused by positively charged plaque on the surface of CAR-T cells is the reason for the tonic signal. Dr. Wu Haitao from EENT Hospital and other experts developed two solutions to reduce the tonic signal during CAR-T therapy development targeting neck tumors. Animal experiments confirmed that these methods can significantly reduce CAR-T cell exhaustion and improve anti-cancer effects.
